Your new company is a well-established care provider headquartered in Townsville with over 200 staff across multiple offices in Queensland. They specialise in disability support and aged care services across Townsville, Charters Towers and Brisbane. They require an Aged Care Service Delivery Manager to join the local team in Brisbane to assist in the running and growth of the Home Care Package side of the business.
Your new role
Your new role will report directly to the Regional Manager and operate as a second-in-charge for the Aged Care division. Your duties:
Assist in the running of the services, which may include managing service budgets, program development and compliance, report and record keeping and vacancy management.
Consulting with clients, carers, families, and guardians to facilitate care assessments, planning, and reviews.
Medication management, clinical assessments, and risk assessments
Complete documentation and administrative reporting
Oversee skill validation of field staff that does not require clinical input
Adhoc Case Management and RN duties as required
What you'll need to succeed
Tertiary qualification in Nursing
Previous experience in Aged Care Case Management
Knowledge of Home Care Packages, Commonwealth Home Support Program and Aged Care Quality Standards is highly desirable
Strong written and computer skills for the creation of plans, programs, reports, and correspondence.
Up-to-date clinical knowledge & skills
Current police check, First Aid & CPR, or ability to obtain
