Our client, Barcaldine Regional Council, has an exciting opportunity for a dedicated Senior Regional Community Care Coordinator to join their team, following recent internal movements within the organisation.
Your new role
The Senior Regional Community Care Coordinator will serve as a primary contact, advocate, and resource within the Community Care Services team. This role involves leading and developing a team to provide high-quality, client-focused support across a broad regional area.
Key Responsibilities:
- Act as the primary contact and advocate for the Community Care Services team, clients, families, and community organisations. Facilitate communication between these groups and guide a team of staff and volunteers dedicated to delivering high-quality, client-centered support.
- Ensure services meet all quality, performance, and outcome metrics by aligning with BRC standards and policies. Conduct assessments and re-assessments to design and review individualised, goal-based Care Plans, collaborating closely with the broader team to implement any necessary changes.
- Work with team leaders to create flexible work plans that outline objectives, activities, and intended outcomes, and oversee the ongoing performance to ensure effectiveness.
- Represent BRC Community Care Services in engagements with government bodies, community networks, and stakeholders to foster strong working relationships. Attend community events, workshops, and training to support team objectives and strengthen connections within the community.
- Drive professional growth within the team by providing structured supervision, coaching, and training to address skill gaps and support team development.
- Utilise systems like Nightingale Notes to perform data entry, track monthly program data, and manage documentation. Analyse program outputs to ensure they meet approved care plans, KPIs, and accreditation standards.
- Assist in preparing funding applications, contribute to budget planning, and manage resources to ensure sustainable service delivery.
What you'll need to succeed
To excel in this role, you will bring:
- A relevant Primary Health or Allied Health qualification (Registered Nurse qualification preferred).
- Current First Aid and CPR certification.
- A valid Queensland class ‘C’ Driver Licence and a current Positive Notice Card or equivalent.
- At least five years of experience in community aged care, with proven expertise in coordinating and managing client care, ideally in a regional or remote setting.
- Comprehensive understanding of relevant care tools and systems, such as Nightingale, My Aged Care, and DEX Data Exchange, with knowledge of Aged Care and NDIS sector standards and legislation.
What you'll get in return
This is a permanent role based in Barcaldine offering $115,000 - $120,000 + locality bonus and super. A relocation package can be negotiated for the right candidate. This role is Monday to Friday, 8.00am to 4.30pm with an accrued RDO fortnightly.
