Hays are partnering with a leading home care provider who are searching for a Clinical Care Coordinator to join their Perth team on a permanent basis.
Your new role
The Clinical Care Coordinator will bring Clinical expertise to the organisation, supporting clients to achieve their goals and maintain independence in their own homes. The main responsibilities of this role include:
• Act as a liaison with clients, families/caregivers and healthcare professionals
• Managing a caseload of home care packages, complex and high needs clients
• Ensure that services meet internal and external quality standards
• Offer support and guidance to clients, families/caregivers and employees to deliver high quality care
• Drive continuous improvement with the organisations strategic objectives
• Actively contribute to enriching the lives of clients living independently in the community
What you'll need to succeed
• Degree qualified Registered Nurse with an active AHPRA registration
• Minimum of 2 years' experience working in case management in aged care or community care
• Strong verbal and written communication skills
• Occasional requirement to participate in a rotating on-call roster
• Willingness to provide clinical care as required
• Knowledge of Aged Care Quality Standards
• Experience managing Home Care Package Budgets desirable
