Your new role
We are looking for a
compassionate and dedicated Care Finder Co-Ordinator to join our team in Mount
Isa! In this role, you will provide one-on-one specialist support to help older
individuals navigate and understand aged
care services and access the support they need. You'll also assist those who
are homeless or at risk of homelessness. This role is
Monday–Friday with some travel involved to service clients in Cloncurry,
Camooweal and Julia Creek as required.
Work Schedule:
- Standard hours: 8:30am – 1:30pm
- Flexible hours based on operational needs
- Extended hours when traveling out of town (7-8 hour days depending on appointments)
Key
Responsibilities:
- Offer Personalised guidance to older individuals to help them understand and access aged care services.
- Conduct home visits for one-on-one consultations with clients.
- Network and build relationships with other agencies.
- Travel to Cloncurry, Camooweal, and Julia Creek as needed.
- What you'll need to succeed
- Blue Card
- Hays Police Check
- Driver’s Licence
- Strong computer skills (emails, data input, reporting)
- Comfortable with home visits and client consultations
- Confident in networking and agency introductions
- Culturally sensitive
- Community service background or previous experience in a similar role (desirable)
